How Do Immune System Problems Lead to Autoimmune Disease?
Autoimmune disease can develop when the immune system loses tolerance for the body’s own cells and tissues. Genetics, infections, and environmental exposures may influence that process, but no single factor explains every case.

The immune system normally protects you from harmful microbes and abnormal cells while avoiding attacks on healthy tissue. Autoimmune disease occurs when this self-protection fails and immune cells, antibodies, or both target the body. The result may be inflammation and tissue damage in one organ or throughout the body.
What does the immune system do?
The immune system is a network of cells, tissues, organs, and proteins. White blood cells detect potential threats, communicate with one another, and coordinate a response. B cells can produce antibodies—proteins that recognize specific targets and help the immune system neutralize them or mark them for removal.
A healthy immune response also depends on immune tolerance: mechanisms that prevent or limit reactions against the body’s own molecules. Tolerance is not simply the ability to tell “good” from “bad”; it is a set of safeguards that helps keep self-reactive immune cells under control.
What is an autoimmune disease?
An autoimmune disease is a condition in which the immune system mistakenly reacts against healthy cells, tissues, or organs. Symptoms vary with the condition and the body area affected. They may include fatigue, joint pain or swelling, skin changes, muscle weakness, numbness, abdominal symptoms, or unexplained fever. These symptoms have many possible causes, so they do not confirm an autoimmune disease by themselves.
How can immune system problems cause autoimmunity?
1. Loss of immune tolerance
Self-reactive immune cells can occasionally arise even in healthy people. Multiple checkpoints normally delete, silence, or restrain them. When these safeguards fail, self-reactive T cells or B cells may remain active. Some B cells may produce autoantibodies—antibodies directed against the body’s own components. The resulting immune response can promote inflammation and, in some diseases, progressive tissue damage.
2. Genetic susceptibility
Certain gene variants can increase susceptibility to particular autoimmune diseases, especially genes involved in immune regulation. Family history can raise risk, but it is not destiny: most autoimmune conditions are influenced by many genes, and inheriting a risk variant does not mean a person will develop disease.
3. Infections and microbes
Infections strongly activate the immune system. In genetically susceptible people, some infections may contribute to the onset or worsening of an autoimmune response through several proposed mechanisms. However, most infections do not cause autoimmune disease, and an infection is rarely the only explanation.
4. Environmental and biologic influences
Researchers continue to study how cigarette smoke, ultraviolet light, certain occupational or chemical exposures, diet, the microbiome, medications, hormones, and other factors influence autoimmune risk. The relevant factors differ by disease. An association does not prove that one exposure caused an individual patient’s condition.

- Type 1 diabetes: immune-mediated destruction of pancreatic beta cells leads to insulin deficiency.
- Multiple sclerosis: immune-mediated damage to myelin and other structures in the central nervous system can affect vision, sensation, strength, coordination, and balance.
- Systemic lupus erythematosus (lupus): may affect the skin, joints, kidneys, blood cells, lungs, heart, brain, or other organs.
- Hashimoto’s disease: immune activity damages the thyroid and commonly leads to hypothyroidism.
- Sjögren’s disease: often affects moisture-producing glands, causing dry eyes and dry mouth, but it can also involve other organs.
How are autoimmune diseases diagnosed?
There is no single test that diagnoses every autoimmune disease. Evaluation usually begins with symptoms, medical and family history, and a physical examination. Depending on the suspected condition, a clinician may order:
- Blood tests for inflammation, blood-cell counts, organ function, or disease-associated autoantibodies.
- Urine testing when kidney or urinary involvement is possible.
- Imaging, pulmonary testing, nerve studies, or other targeted evaluations.
- A biopsy when examination of tissue is needed to clarify the diagnosis.
Autoantibody results must be interpreted in context. A positive test does not always mean disease is present, and a negative test does not exclude every autoimmune condition. Testing should be guided by symptoms and the suspected diagnosis rather than used as a broad screening panel without a clinical reason.
How are autoimmune diseases treated?
Treatment depends on the diagnosis, disease activity, organs involved, and the patient’s overall health. Goals may include relieving symptoms, reducing inflammation, limiting harmful immune activity, replacing a missing hormone or other substance, preventing organ damage, and maintaining function. Options may include anti-inflammatory medicines, corticosteroids, disease-modifying drugs, targeted biologic therapies, physical or occupational therapy, and condition-specific treatments. Because some therapies suppress immune activity, monitoring for infections and other adverse effects may be necessary.
When should you seek medical care?
